Pet Nutrition Pro Client Intake & Consent Agreement

This agreement applies to services provided through Pet Nutrition Pro, operated by Macaulay Rider. Throughout this document, Macaulay Rider will be referred to as “Mac Gray.” All consultations, educational services, and wellness guidance referenced in this agreement are provided by Mac Gray through Pet Nutrition Pro.

1. Credentials

Mac Gray (Macaulay Rider) is a Board Certified Holistic Pet Coach, Certified Canine Nutritionist, Certified Feline Nutritionist, Certified Commercial Pet Food Educator, Certified Raw Pet Food Formulator, Certified Pet Herbalism Specialist, and Certified Pet Homeopathy Specialist.

She is also affectionately known by clients as “The Holistic Dog Whisperer” and an Animal Naturopath, reflecting her holistic approach to supporting pet wellness through nutrition, lifestyle guidance, and root cause exploration.

Services provided through Pet Nutrition Pro are educational, wellness-focused, and supportive in nature and are intended to help pet parents better understand nutrition, lifestyle factors, and holistic wellness strategies for their pets.

2. Scope of Practice

Mac Gray provides holistic pet wellness education and guidance, which may include education and recommendations related to:

• Pet nutrition and feeding practices
• Supplement education
• Herbal wellness support
• Homeopathy and Bach flower essences
• Environmental wellness factors including light exposure, hydration, air quality, and lifestyle influences
• Root cause exploration and pattern recognition from a holistic wellness perspective

Mac Gray does not diagnose, treat, cure, or prevent disease. Mac Gray is not a veterinarian, and services provided do not replace veterinary care, diagnosis, treatment, prescription medications, or emergency services.

Clients are strongly encouraged to maintain an ongoing relationship with a licensed veterinarian for medical concerns, diagnostics, laboratory testing, and emergency care.

3. Educational Nature of Services

All consultations, recommendations, reports, wellness plans, and educational materials provided are intended for informational and educational purposes only.

Holistic wellness guidance may include forward-looking statements or supportive strategies intended to help pet parents explore potential wellness approaches. These statements should not be interpreted as medical claims or guarantees of outcomes.

Pet parents understand that:

• Holistic wellness approaches are individualized
• Responses may vary from pet to pet
• No results or outcomes can be guaranteed

Clients acknowledge that they are responsible for conducting their own research and consulting with their veterinarian before introducing any new food, supplement, herb, homeopathic preparation, or wellness practice.

4. Consent to Receive Holistic Wellness Guidance

By signing this agreement, the client acknowledges and consents to receiving:

• Educational wellness guidance and consultation services
• Individualized wellness recommendations
• Written wellness plans or PDF protocols when applicable
• Holistic pattern recognition and root cause exploration discussions

Clients understand that all decisions regarding their pet’s care remain their sole responsibility.

5. Client Responsibility

The client agrees to provide accurate and complete information regarding their pet’s health history, diet, medications, supplements, and veterinary care to the best of their knowledge.

The client understands that incomplete or inaccurate information may impact the usefulness or safety of educational guidance provided.

The client acknowledges that implementation of any recommendations is voluntary and at their own discretion.

6. Financial Responsibility & Payment Policy

The client agrees to pay all fees associated with services provided by Pet Nutrition Pro.
Payment is required at the time of booking unless otherwise stated.

Fees compensate for:

• Professional consultation time
• Education and expertise
• Case review and analysis
• Preparation of individualized wellness plans or written reports

Due to the time, preparation, and intellectual work involved in consultations and wellness plan development:

All services are non-refundable once the consultation has occurred or written materials have been delivered.

Missed appointments or late cancellations may not be eligible for refund.

7. Limitation of Liability

The client voluntarily assumes full responsibility for decisions made regarding their pet’s care.
The client agrees that Pet Nutrition Pro and Mac Gray shall not be held liable for outcomes resulting from the use or misuse of educational information, guidance, or recommendations provided.
All actions taken based on educational guidance are the sole responsibility of the pet owner.

8. Voluntary Consent & Acknowledgement

By signing below, the client confirms that they:

• Have read and understand this agreement
• Understand the scope and limitations of services provided
• Understand that services are educational and not veterinary medicine
• Acknowledge that Mac Gray is not a veterinarian
• Accept responsibility for decisions regarding their pet s care
• Accept financial responsibility for services rendered
• Consent to receiving holistic wellness guidance and recommendations
